Critical thinking skills for kids are crucial: The lure of Superbowl alcohol ads

Here’s today’s COMMENT FROM AN OLD FART: Aside from the action on the field at the Superbowl, many folks tune into the game because of the half-time show and the over-the-top commercials. Critical thinking skills are lacking in many adults. Chldren not only may lack critical thinking skills, but may make poor choices because of their lack of maturity. Yolanda Evans, MD, MPH writes in the Seattle Children’s Hospital article, Alcohol Ads and Teen Drinking:

A recent article in the journal Pediatrics looked at 4,000 students in 7th grade and asked about alcohol use and alcohol ads on TV. They surveyed the teens through 10th grade. Though the number of teens participating decreased over time, they found some scary results. For both boys and girls, increasing exposure to alcohol ads over time and liking what they saw was associated with more alcohol use from 7th to 10th grade.  They also assessed alcohol related problems, like trouble with school, and found a significant association among boys and ads.

These results show that ads can affect behavior. So what can a parent do?

  1. limit screen time and exposure to mature subject matter. The American Academy of Pediatrics recommends limiting screen time to 2 hours a day. This helps decrease exposure, but also encourages teens to do something active with their time.

  2. Use the ads as an opportunity to talk about drug use. Let teens know that what they see in these ads is not reality. Talk about the dangers of alcohol. Short term effects include difficulty in school, possible alcohol poisoning, increased risk taking and long term include health problems like liver and heart disease.

  3. Set limits and talk about consequences before you need them. See our posts on the ‘free phone call‘ and ‘ground rules.’ Talk with your teen about expectations of their behavior and let them help decide on consequences if they break the rules.

    Abstract

OBJECTIVE: This study used prospective data to test the hypothesis that exposure to alcohol advertising contributes to an increase in underage drinking and that an increase in underage drinking then leads to problems associated with drinking alcohol.

METHODS: A total of 3890 students were surveyed once per year across 4 years from the 7th through the 10th grades. Assessments included several measures of exposure to alcohol advertising, alcohol use, problems related to alcohol use, and a range of covariates, such as age, drinking by peers, drinking by close adults, playing sports, general TV watching, acculturation, parents’ jobs, and parents’ education.

RESULTS: Structural equation modeling of alcohol consumption showed that exposure to alcohol ads and/or liking of those ads in seventh grade were predictive of the latent growth factors for alcohol use (past 30 days and past 6 months) after controlling for covariates. In addition, there was a significant total effect for boys and a significant mediated effect for girls of exposure to alcohol ads and liking of those ads in 7th grade through latent growth factors for alcohol use on alcohol-related problems in 10th grade.

CONCLUSIONS: Younger adolescents appear to be susceptible to the persuasive messages contained in alcohol commercials broadcast on TV, which sometimes results in a positive affective reaction to the ads. Alcohol ad exposure and the affective reaction to those ads influence some youth to drink more and experience drinking-related problems later in adolescence.

Underage Drinking

Alcohol use by persons under age 21 years is a major public health problem.1 Alcohol is the most commonly used and abused drug among youth in the United States, more than tobacco and illicit drugs. Although drinking by persons under the age of 21 is illegal, people aged 12 to 20 years drink 11% of all alcohol consumed in the United States.2 More than 90% of this alcohol is consumed in the form of binge drinks.2 On average, underage drinkers consume more drinks per drinking occasion than adult drinkers.3 In 2008, there were approximately 190,000 emergency rooms visits by persons under age 21 for injuries and other conditions linked to alcohol.4

Drinking Levels among Youth

The 2009 Youth Risk Behavior Survey5 found that among high school students, during the past 30 days

  • 42% drank some amount of alcohol.

  • 24% binge drank.

  • 10% drove after drinking alcohol.

  • 28% rode with a driver who had been drinking alcohol.

Other national surveys indicate

  • In 2008 the National Survey on Drug Use and HealthExternal Web Site Icon reported that 28% of youth aged 12 to 20 years drink alcohol and 19% reported binge drinking.6

  • In 2009, the Monitoring the Future SurveyExternal Web Site Icon reported that 37% of 8th graders and 72% of 12th graders had tried alcohol, and 15% of 8th graders and 44% of 12th graders drank during the past month.7

Consequences of Underage Drinking

Youth who drink alcohol1, 3, 8 are more likely to experience

  • School problems, such as higher absence and poor or failing grades.

  • Social problems, such as fighting and lack of participation in youth activities.

  • Legal problems, such as arrest for driving or physically hurting someone while drunk.

  • Physical problems, such as hangovers or illnesses.

  • Unwanted, unplanned, and unprotected sexual activity.

  • Disruption of normal growth and sexual development.

  • Physical and sexual assault.

  • Higher risk for suicide and homicide.

  • Alcohol-related car crashes and other unintentional injuries, such as burns, falls, and drowning.

  • Memory problems.

  • Abuse of other drugs.

  • Changes in brain development that may have life-long effects.

  • Death from alcohol poisoning.

In general, the risk of youth experiencing these problems is greater for those who binge drink than for those who do not binge drink.8

The issue is whether children in a “captive” environment have the maturity and critical thinking skills to evaluate the information contained in the ads. Advertising is about creating a desire for the product, pushing a lifestyle which might make an individual more prone to purchase products to create that lifestyle, and promoting an image which might make an individual more prone to purchase products in pursuit of that image. Many girls and women have unrealistic body image expectations which can lead to eating disorders in the pursuit of a “super model” image. What the glossy magazines don’t tell young women is the dysfunctional lives of many “super models” which may involve both eating disorders and substance abuse. The magazines don’t point out that many “glamor girls” are air-brushed or photo-shopped and that they spend hours on professional make-up and professional hairstyling in addition to having a personal trainer and stylist. Many boys look at the buff bodies of the men in the ads and don’t realize that some use body enhancing drugs. In other words, when presented with any advertising, people must make a determination what to believe. It is easy for children to get derailed because of peer pressure in an all too permissive society. Parents and schools must teach children critical thinking skills and point out often that the picture presented in advertising is often as close to reality as the bedtime fairy tail. Reality does not often involve perfection, there are warts.

Moi is a ‘bus chick’ and rides the bus all over Seattle or she walks. She is registered with the two pay-by-use car programs, but rarely uses them. The bus takes moi everywhere she needs to go. The bus tunnel runs under downtown Seattle and is deep underground. To reach the surface, one takes an escalator, at the deepest points of tunnel, to the street. Today moi was leaving the tunnel and riding the escalator to the street. There were several people who went past moi walking up the escalator. They shaved maybe 30 seconds off the escalator ride. Than got moi thinking about the speed of contemporary life. Today’s deposit into the ‘Joy Jar’ is a liveable pace of life.

Life is made up of special moments which make it worth living. There are many cherished moments that are missed due to the stress and fast pace of daily living. We must slow down and remember how precious it is to be alive and to love!!!!

GAO report: Children’s mental health services are lacking

Both the culture and the economy are experiencing turmoil. For some communities, the unsettled environment is a new phenomenon, for other communities, children have been stressed for generations. According to the article, Understanding Depression which was posted at the Kids Health site:

One strategy in helping children to succeed is to recognize and treat depression.

How Common Is Depression In Children?      

According to Mary H. Sarafolean, PhD in the article, Depression In School Age Children and Adolescents

In general, depression affects a person’s physical,  cognitive, emotional/affective, and motivational well-being, no matter  their age. For example, a child with depression between the ages of 6 and 12 may exhibit fatigue, difficulty with schoolwork, apathy and/or a lack of motivation. An adolescent or teen may be oversleeping, socially isolated, acting out in self-destructive ways and/or have a sense of hopelessness.

Prevalence and Risk Factors

While only 2 percent of pre-teen school-age children and 3-5 percent of teenagers have clinical depression, it is the most common diagnosis of children in a clinical setting (40-50 percent of diagnoses). The lifetime risk  of depression in females is 10-25 percent and in males, 5-12 percent. Children and teens who are considered at high risk for depression disorders include:

* children referred to a mental health provider for school problems
* children with medical problems
* gay and lesbian adolescents
* rural vs. urban adolescents
* incarcerated adolescents
* pregnant adolescents
* children with a family history of depression    

If you or your child has one or more of the risk factors and your child is exhibiting symptoms of prolonged sadness, it might be wise to have your child evaluated for depression. 

How to Recognize Depression In Your Child?

Signs and symptoms of depression in children include:       

* Irritability or anger
* Continuous feelings of sadness, hopelessness
* Social withdrawal
* Increased sensitivity to rejection
* Changes in appetite — either increased or decreased
* Changes in sleep — sleeplessness or excessive sleep
* Vocal outbursts or crying
* Difficulty concentrating
* Fatigue and low energy
* Physical complaints (such as stomachaches, headaches) that do not respond to
treatment
* Reduced  ability to function during events and activities at home or with friends, in school, extracurricular activities, and in other hobbies or  interests

* Feelings of worthlessness or guilt
* Impaired thinking or concentration
* Thoughts of death or suicide        

Not all children have all of these symptoms. In fact, most will  display different symptoms at different times and in different settings.  Although some children may continue to function reasonably well in  structured environments, most kids with significant depression will  suffer a noticeable change in social activities, loss of interest in  school and poor academic performance, or a change in appearance.  Children may also begin using drugs or alcohol,
especially if they are  over the age of 12.

Joy Resmovits reported in the article, Mental Health Care For Kids Severely Lacking, Says GAO which was posted at Huffington Post:

“Most children whose emotions or behavior, as reported by their parent or guardian, indicated a potential need for a mental health service did not receive any services within the same year,” the GAO wrote.

The report comes after Sen. Tom Harkin (D-Iowa), Rep. Rosa DeLauro (Conn.), and Rep. Lucille Roybal-Allard (Calif.) requested that the GAO look into how psychotropic drugs affect the long-term development of kids who grow up in foster care. While the report is very specific in its scope, it’s sure to be a relevant piece of evidence as the Obama administration formulates policy to deal with the ramifications of the Newtown, Conn. elementary school shooting. The shooting has sparked a nationwide debate on gun control, but it has also directed America’s attention to the state of its mental health care system.

Here are some findings:

  • On average, 6.2 percent of noninstitutionalized children in Medicaid and 4.8 percent of privately insured kids received psychotropic medications.

  • 30 percent of foster children who might have required mental health care didn’t receive them over the last year.

  • Most kids outside the foster care system whose behavior displayed red flags didn’t receive mental care services.

  • Many kids who got psychotropic medication didn’t get counseling or therapy to complete the care.

What GAO Found

An annual average of 6.2 percent of noninstitutionalized children in Medicaid nationwide and 4.8 percent of privately insured children took one or more psychotropic medications, according to GAO’s analysis of 2007-2009 data from the Department of Health and Human Services’ (HHS) Medical Expenditure Panel Survey (MEPS). MEPS data also showed that children in Medicaid took antipsychotic medications (a type of psychotropic medication that can help some children but has a risk of serious side effects) at a relatively low rate–1.3 percent of children–but that the rate for children in Medicaid was over twice the rate for privately insured children, which was 0.5 percent. In addition, MEPS data showed that most children whose emotions or behavior, as reported by their parent or guardian, indicated a potential need for a mental health service did not receive any services within the same year. The Centers for Medicare & Medicaid Services (CMS) and many states have initiatives under way to help ensure that children receive appropriate mental health treatments. However, CMS’s ability to monitor children’s receipt of mental health services is limited because CMS does not collect information from states on whether children in Medicaid have received services for which they were referred. GAO recommended in 2011 that CMS identify options for collecting such data from state Medicaid programs. Findings in this report underscore the continued importance of CMS’s monitoring of children’s receipt of mental health services.

HHS’s Administration for Children and Families (ACF) reported that 18 percent of foster children were taking psychotropic medications at the time they were surveyed, although utilization varied widely by the child’s living arrangement. ACF also reported that 30 percent of foster children who may have needed mental health services did not receive them in the previous 12 months. HHS agencies are taking steps to promote appropriate mental health treatments for foster children, such as by sending information to states on psychotropic medication oversight practices.

HHS’s National Institutes of Health spent an estimated $1.2 billion on over 1,200 children’s mental health research projects during fiscal years 2008 through 2011. Most of the funding–$956 million–was awarded by the National Institute of Mental Health, with more research projects studying psychosocial therapies than psychotropic medications. Other HHS agencies spent about $16 million combined on children’s mental health research during this period.

HHS reviewed a draft of this report and provided technical comments, which GAO incorporated as appropriate.

Why GAO Did This Study

Experts have concerns that children with mental health conditions do not always receive appropriate treatment, including concerns about appropriate use of psychotropic medications (which affect mood, thought, or behavior) and about access to psychosocial therapies (sessions with a mental health provider). These concerns may be compounded for low-income children in Medicaid and children in foster care (most of whom are covered by Medicaid)–populations who may be at higher risk of mental health conditions. Within HHS, CMS oversees Medicaid, and ACF supports state child welfare agencies that coordinate health care for foster children.

GAO was asked to provide information on children’s mental health. This report examines (1) the use of psychotropic medications and other mental health services for children in Medicaid nationwide, and related CMS initiatives; (2) HHS information on the use of psychotropic medications and other mental health services for children in foster care nationwide, and related HHS initiatives; and (3) the amount HHS has invested in research on children’s mental health.

GAO analyzed data from HHS’s MEPS –a national household survey on use of medical services–from 2007 through 2009 for children covered by Medicaid and private insurance. GAO reviewed two recent ACF foster care reports with data from a national survey conducted during 2008 through 2011. GAO analyzed data from HHS agencies that conduct or fund research and interviewed HHS officials and children’s mental health providers, researchers, and advocates.

Concerns Remain about Appropriate Services for Children in Medicaid and Foster Care GAO-13-15, Dec 10, 2012

Study: Current therapies may not be effective in preventing teen suicide, but targeted treatment helps

People of all ages may have feelings of profound sadness, loss, and depression. There is no one on earth, despite what the ads attempt to portray, who lives a perfect life. Every life has flaws and blemishes, it is just that some cope better than others. For every person who lives to a ripe old age, during the course of that life they may encounter all types of loss from loss of a loved one through death, divorce or desertion, loss of job, financial reverses, illness, dealing with A-holes and twits, plagues, pestilence, and whatever curse can be thrown at a person. The key is that they lived THROUGH whatever challenges they faced AT THAT MOMENT IN TIME. Woody Allen said something like “90% of life is simply showing up.” Let moi add a corollary, one of the prime elements of a happy life is to realize that whatever moment you are now in, it will not last forever and that includes moments of great challenge. A person does not have to be religious to appreciate the story of Job. The end of the story is that Job is restored. He had to endure much before the final victory, though.

Why Do Teens Attempt Suicide?

Suicides among young people continue to be a serious problem. Each year in the U.S., thousands of teenagers commit suicide. Suicide is the third leading cause of death for 15-to-24-year-olds, and the sixth leading cause of death for 5-to-14-year-olds.

Teenagers experience strong feelings of stress, confusion, self-doubt, pressure to succeed, financial uncertainty, and other fears while growing up. For some teenagers, divorce, the formation of a new family with step-parents and step-siblings, or moving to a new community can be very unsettling and can intensify self-doubts. For some teens, suicide may appear to be a solution to their problems and stress.

Sometimes, people see suicide as an answer to their problems. All of us must stress that suicide is always the WRONG answer to what in all likelihood is a transitory situation.

What are the Warning Signs of Suicide?

According to Teen’s Health there are some suicide warning signs

There are often signs that someone may be thinking about or planning a suicide attempt. Here are some of them:

talking about suicide or death in general

talking about “going away”

referring to things they “won’t be needing,” and giving away possessions

talking about feeling hopeless or feeling guilty

pulling away from friends or family and losing the desire to go out

having no desire to take part in favorite things or activities

having trouble concentrating or thinking clearly

experiencing changes in eating or sleeping habits

engaging in self-destructive behavior (drinking alcohol, taking drugs, or cutting, for example)

These are signs that indicate a person may be depressed.

According to JaredStory.com the primary cause of suicide is depression.

# 1 CAUSE OF SUICIDE: UNTREATED DEPRESSION

It can be very hard to diagnose depression. There are many different kinds of depression and not all people will have the same symptoms, or have them to the same degree. Here are some symptoms to watch for and if they last more than a few weeks, a doctor or psychiatrist should be consulted.

Persistent sad or “empty” mood

Feeling hopeless, helpless, worthless

pessimistic and or guilty

Substance abuse

Fatigued or loss of interest in ordinary activities

Disturbances in eating and sleeping patterns

Irritability, increased crying, anxiety and panic attacks, (Post Traumatic Stress Disorder)

Difficulty concentrating, remembering or making decisions

Thoughts of suicide; suicide plans or attempts

Persistent physical symptoms or pains that do not respond to treatment

The site also lists events that might trigger depression in a person.

A death of a family member or close friend – which could include a fellow student from school

An assault, car accident or painful physical event – which could include physical bullying

Mental, or emotional event – which could include non-physical bullying

Marriage breakup, or love lost suddenly – which could include “breaking up” with a girlfriend or boyfriend

Constant physical, mental, or emotional pain that goes on for a length of time – which includes constant bullying that is not intervened, resolved or stopped entirely

Major Financial setback – which includes a teenager who may have lost a job

Something “embarrassing” happens – as an example; getting kicked off a football team or a public insult by a teacher or popular student; bullying

Failing an important exam a school – not a normal trigger unless the exam was life changing and the individual is under a lot of stress

A best friend moves out of town – especially true for teenagers who are being bullied and have very few friends as it is

If you notice these signs, the key is to get help for yourself or a friend. The type of treatment will depend upon the underlying symptoms.

Benedict Carey reports in the New York Times article, Study Questions Effectiveness of Therapy for Suicidal Teenagers:

Most adolescents who plan or attempt suicide have already received at least some mental health treatment, raising questions about the effectiveness of current approaches to helping troubled youths, according to the largest in-depth analysis to date of suicidal behaviors in American teenagers.

The study, in the journal JAMA Psychiatry, found that 55 percent of suicidal teenagers had received some therapy before they thought about suicide, planned it or tried to kill themselves, contradicting the widely held belief that suicide is due in part to a lack of access to treatment.

The findings, based on interviews with a nationwide sample of more than 6,000 teenagers and at least one parent of each, linked suicidal behavior to complex combinations of mood disorders like depression and behavior problems like attention-deficit and eating disorders, as well as alcohol and drug abuse.

The study found that about one in eight teenagers had persistent suicidal thoughts at some point, and that about a third of those who had suicidal thoughts had made an attempt, usually within a year of having the idea.

Previous studies have had similar findings, based on smaller, regional samples. But the new study is the first to suggest, in a large nationwide sample, that access to treatment does not make a big difference. ..

Over all, about one-third of teenagers with persistent suicidal thoughts went on to make an attempt to take their own lives.

Almost all of the suicidal adolescents in the study qualified for some psychiatric diagnosis, whether depression, phobias or generalized anxiety disorder. Those with an added behavior problem — attention-deficit disorder, substance abuse, explosive anger — were more likely to act on thoughts of self-harm, the study found.

Doctors have tested a range of therapies to prevent or reduce recurrent suicidal behaviors, with mixed success. Medications can ease depression, but in some cases they can increase suicidal thinking. Talk therapy can contain some behavior problems, but not all.

One approach, called dialectical behavior therapy, has proved effective in reducing hospitalizations and suicide attempts in, among others, people with borderline personality disorder, who are highly prone to self-harm.

But suicidal teenagers who have a mixture of mood and behavior issues are difficult to reach. In one 2011 study, researchers at George Mason University reduced suicide attempts, hospitalizations, drinking and drug use among suicidal adolescent substance abusers. Energy drinks may pose a danger

The American Academy of Pediatrics is reporting at its site, Healthy Children.Org in the study, Energy Drinks Can Harm Children

Energy drinks may pose a risk for serious adverse health effects in some children, especially those with diabetes, seizures, cardiac abnormalities or mood and behavior disorders.

A new study, “Health Effects of Energy Drinks on Children, Adolescents, and Young Adults,” in the March issue of Pediatrics (published online Feb. 14), determined that energy drinks have no therapeutic benefit to children, and both the known and unknown properties of the ingredients, combined with reports of toxicity, may put some children at risk for adverse health events.

Youth account for half of the energy drink market, and according to surveys, 30 percent to 50 percent of adolescents report consuming energy drinks. Typically, energy drinks contain high levels of stimulants such as caffeine, taurine, and guarana, and safe consumption levels have not been established for most adolescents. Because energy drinks are frequently marketed to athletes and at-risk young adults, it is important for pediatric health care providers to screen for heavy use both alone and with alcohol, and to educate families and children at-risk for energy drink overdose, which can result in seizures, stroke and even sudden death.

Several deaths have been attributed to energy drinks.

The Washington Post reports in the articleEnergy drink popularity booms at college, despite health concerns:

A 2008 study of undergraduates at a large public university found that 39 percent of students had consumed at least one energy drink in the past month, with considerably higher rates for males and white students. The study, funded with a National Institute on Drug Abuse grant, noted that energy drink marketing tactics are “similar to those used to sell tobacco and alcohol to youths….”

Red Bull, which hit the country in the late 1990s, is credited with creating this industry using a Thai recipe. Today there are hundreds of energy drinks on the market, ranging from 1.93-ounce 5-Hour Energy shots to 32-ounce cans of Monster. Even Starbucks has gotten into the game, producing sparkling energy drinks and canned espresso beverages.

That proliferation has intensified debate about a long-standing question: Are energy drinks safe?

The focus of that question is often one of the main ingredients: caffeine. As more young people consume energy drinks, more problems are occurring.

Daniel J. DeNoon of WebMD Health News reports in the article, More Deaths, Illness Linked to Energy Drinks which was reviewed by Louise Chang, MD reports:

Nov. 16, 2012 — The FDA has posted adverse-event reports for two more energy drinks: 40 illnesses and five deaths linked to Monster Energy, and 13 illnesses and two lasting disabilities linked to Rockstar Energy.

The new reports follow this week’s revelation of FDA reports linking 92 illnesses and 13 deaths to 5-Hour Energy shots. The FDA previously said it was investigating the deaths linked to Monster Energy.

These adverse-event reports (AERs) are filed by patients, families, or doctors. They simply warn that the products might have harmed someone — but they do not prove that the product caused harm. The FDA can remove a product from the market only when investigation shows that the product causes harm when used according to the product label.

“If we find a relationship between consumption of the product and harm, FDA will take appropriate action to reduce or eliminate the risk,”  FDA public information officer Shelly Burgess says.

Moreover, the reports do not offer details on any underlying medical conditions that may have led to product-related illnesses.

The reports, some dating back to 2004, are not a complete inventory of all events that product users may have suffered. Most people, and many doctors, do not know how to file these reports or do not get around to filing them. And even when a product actually causes an illness, a user or doctor may not associate the product with the illness.

The new reports detail the events suffered by users of 5-Hour, Monster, and Rockstar energy drinks. These include:

  • Deaths due to heart attack or suicide linked to 5-Hour Energy
  • A miscarriage linked to 5-Hour Energy
  • Convulsions, life-threatening fear, deafness, and hemorrhage linked to 5-Hour Energy
  • Deaths due to heart attack or loss of consciousness linked to Monster Energy drink
  • Hospitalization due to irregular heartbeat, severe diarrhea, migraine, psychotic disorder, heart attack, and/or vomiting linked to Monster Energy drink
  • Disability from irregular heartbeat or stroke linked to Rockstar Energy drink
  • Hospitalization due to psychotic disorder, increased heart rate, or loss of consciousness linked to Rockstar Energy drink

Daytime Sleepiness

Many people depend on coffee to start their day. Caffeine boosts your energy and causes you to feel more alert and awake. Since these effects wear off after a few hours, you may experience a mid-afternoon lull shortly after lunch. Energy drinks can provide the extra oomph that you seek in order to get through your day. These energy drinks are often marketed towards young adults who have responsibilities in addition to work, such as family obligations, or aspirations to further their education.

Lack of Sleep

The regular use of energy drinks may be indicative of an underlying condition such as fatigue or insomnia, according to registered dietitian Kara Mitchell from Duke University. Fatigue is a symptom of a multitude of mental and physical health conditions. Fatigue caused by insomnia or lack of sleep is treatable. However, frequent use of energy drinks may mask the underlying condition. Energy drinks may also cause adverse health conditions such as irritability and high blood pressure, according to Mitchell.

Alcohol

The majority of energy drinks do not contain alcohol. Mixing alcohol with energy drinks is not uncommon among college students and young adults. The idea is that caffeine will reduce the sedative effects of alcohol and make you more alert. Researchers assessed the attention and reaction times of young adult drinkers between the ages of 21 and 30, after drinking energy drinks with alcohol. This study was published in the February 2011 issue of “Addiction,” and found an association between alcohol and impairments in attention and reaction. However, the addition of caffeine did not mitigate these impairments.

Increase Performance

Energy drinks and sports drinks, like Gatorade and Powerade, are not interchangeable. As such, they may be kept in separate aisles at supermarkets. Dr. Edward Laskowski of the Mayo Clinic suggests drinking water or sports drinks when you exercise, not energy drinks. One reason is that sports drinks replenish fluids and electrolytes, whereas energy drinks do not. Another reason is that the caffeine in energy drinks may cause restlessness, headaches, nausea and tremors. Excessive use is associated with chest pains, seizures, heart attack and even sudden cardiac death.

Volunteers see greater benefits than those they are serving

One study followed women with multiple sclerosis (MS) who volunteered as peer supporters to other patients. They received training in compassionate listening techniques and called the patients to talk and listen for 15 minutes at a time. The study followed the volunteers for three years and found that they had increased self-esteem, self-acceptance, satisfaction, self-efficacy, social activity, and feelings of mastery. The positive outcomes for the volunteers were even greater than for the patients they were helping.

Compassion fosters happiness, but being sacrificial reduces well-being

Being kind and compassionate is linked to greater happiness, greater levels of physical activity well into old age, and longevity. One important caveat: if people get overextended and overwhelmed by helping tasks, as can happen with people who are caregivers to family members, their health and quality of life can rapidly decline. It seems being generous from an abundance of time, money, and energy can promote well-being; but being sacrificial quickly lowers well-being. A gift should be an act of altruism, otherwise it is a form of extortion. People who pay $7 for a cup of coffee are within their rights and have their free will to do so. They probably would be happier being more modest.

Brian Toporek writes in the Education Week article, Study: Muscle-Enhancing Behaviors Increasingly Common Among Teens:

Both teenage boys and girls are engaging in muscle-enhancing behaviors far more than previously known, according to a study published online today in the journal Pediatrics.

As large, lean, muscular male body images have risen in popularity in Western culture, so too has teenage boys’ dissatisfaction with their own bodies, the study suggests. Some boys thus decide to engage in muscle-enhancing behaviors to shape their bodies like the ones being presented to them in the media.

For this study, three researchers from the University of Minnesota and Columbia University examined data from 2,793 youths (with a mean age of 14.4) at 20 urban middle and high schools taken during the 2009-10 school year. The researchers set out to determine the prevalence of five specific muscle-enhancing behaviors: changing eating habits to increase muscle size, increasing exercise, the use of protein powder, the use of steroids, and the use of other muscle-enhancing substances.

Nearly 70 percent of the boys in the study (897 of 1,307 total) reported having changed their eating habits in order to increase their muscle size or tone within the past 12 months, and more than 90 percent of boys increased their amount of exercise to achieve the same goal.

More than 40 percent of boys reported that they often exercised more to boost their muscle mass or tone, while 39.1 percent sometimes did, and 11.3 rarely did. Only 8.8 percent of boys never did, according to the study.

While changing eating habits and exercising more could each be considered healthy habits, many boys engaged in unhealthy behaviors, too. More than one-third of the boys in the study reported using protein powders or shakes, 5.9 percent reported using steroids, and 10.5 percent reported using some other muscle-enhancing substance.

On the female side, more than 60 percent of girls reported changing their eating habits to increase muscle size or tone, and more than 80 percent of girls exercised more for the same reason. More than 20 percent of girls reported using protein powders or shakes, 4.6 percent reported using steroids, and 5.5 percent reported using other muscle-enhancing substances…

Generva Pittman of Reuters writes about steroids in the article, One in 20 youth has used steroids to bulk up: study:

Anabolic steroids are synthetic versions of testosterone, the male sex hormone. Steroids are prescribed legally to treat conditions involving hormone deficiency or muscle loss, but when they’re used for non-medical purposes, it’s typically at much higher doses, according to the National Institute on Drug Abuse.

In those cases, steroids can cause mood swings – sometimes known as roid rage – and for adolescents, stunted growth and accelerated puberty.

Anabolic steroids have become pervasive in professional sports, including baseball, football and boxing. (Another example of performance-enhancing drug use is “blood doping” with erythropoietin or EPO, which is behind the Lance Armstrong cycling controversy that caused him to be stripped of his Tour de France titles last month.)

Experts have worried that the drive to get ahead of competitors at any cost could trickle down to college and high school athletes, as well.

OBJECTIVE: Media images of men and women have become increasingly muscular, and muscle-enhancing techniques are available to youth. Identifying populations at risk for unhealthy muscle-enhancingbehaviors is of considerable public health importance. The current study uses a large and diverse population-based sample of adolescents to examine the prevalence of muscle-enhancing behaviors and differences across demographic characteristics, weight status, and sports team involvement.

METHODS: Survey data from 2793 diverse adolescents (mean age = 14.4) were collected at 20 urban middle and high schools. Use of 5 muscle-enhancing behaviors was assessed (changing eating, exercising, protein powders, steroids and other muscle-enhancing substances), and a summary score reflecting use of 3 or more behaviors was created. Logistic regression was used to test for differences in each behavior across age group, race/ethnicity, socioeconomic status, BMI category, and sports team participation.

RESULTS: Muscle-enhancing behaviors were common in this sample for both boys and girls. For example, 34.7% used protein powders or shakes and 5.9% reported steroid use. Most behaviors were significantly more common among boys. In models mutually adjusted for all covariates, grade level, Asian race, BMI category, and sports team participation were significantly associated with the use of muscle-enhancing behaviors. For example, overweight (odds ratio = 1.45) and obese (odds ratio = 1.90) girls had significantly greater odds of using protein powders or shakes than girls of average BMI.

CONCLUSIONS: The use of muscle-enhancing behaviors is substantially higher than has been previously reported and is cause for concern. Parents have more influence on their children’s values and beliefs than most are willing to exercise. You need to support your children’s dreams, not yours.

Because people have free will, even the best parents will have children who make mistakes. Some so identified “progressives” will attribute this lapse not to individual free will, but the fact that the message of morality is a failure. It is not. Study: Early stress in girls may be the source of later anxiety

Waisman Laboratory for Brain Imaging and Behavior published a study which looks at the effects of stress on girls.

Science Daily is reporting in the article, Early Stress May Sensitize Girls’ Brains for Later Anxiety:

High levels of family stress in infancy are linked to differences in everyday brain function and anxiety in teenage girls, according to new results of a long-running population study by University of Wisconsin-Madison scientists.

The study highlights evidence for a developmental pathway through which early life stress may drive these changes. Here, babies who lived in homes with stressed mothers were more likely to grow into preschoolers with higher levels of cortisol, a stress hormone. In addition, these girls with higher cortisol also showed less communication between brain areas associated with emotion regulation 14 years later. Last, both high cortisol and differences in brain activity predicted higher levels of adolescent anxiety at age 18.

The young men in the study did not show any of these patterns.

“We wanted to understand how stress early in life impacts patterns of brain development which might lead to anxiety and depression,” says first author Dr. Cory Burghy of the Waisman Laboratory for Brain Imaging and Behavior. “Young girls who, as preschoolers, had heightened cortisol levels, go on to show lower brain connectivity in important neural pathways for emotion regulation — and that predicts symptoms of anxiety during adolescence….”

The current paper has its roots back in 1990 and 1991, when 570 children and their families enrolled in the Wisconsin Study of Families and Work (WSFW). All of the children were born in either Madison or Milwaukee. Dr. Marilyn Essex, a UW professor of psychiatry and co-director of the WSFW, said the initial goal was to study the effects of maternity leave, day care and other factors on family stress. Over the years, the study has resulted in important findings on the social, psychological, and biological risk factors for child and adolescent mental health problems. Subjects are now 21 and 22 years old, and many continue to participate.

For the current study, Burghy and Birn used fcMRI to scan the brains of 57 subjects — 28 female and 29 male — to map the strength of connections between the amygdala, an area of the brain known for its sensitivity to negative emotion and threat, and the prefrontal cortex, often associated with helping to process and regulate negative emotion. Then, they looked back at earlier results and found that girls with weaker connections had, as infants, lived in homes where their mothers had reported higher general levels of stress — which could include symptoms of depression, parenting frustration, marital conflict, feeling overwhelmed in their role as a parent, and/or financial stress. Abstract

Early life stress (ELS) and function of the hypothalamic-pituitary-adrenal axis predict later psychopathology. Animal studies and cross-sectional human studies suggest that this process might operate through amygdala–ventromedial prefrontal cortex (vmPFC) circuitry implicated in the regulation of emotion. Here we prospectively investigated the roles of ELS and childhood basal cortisol amounts in the development of adolescent resting-state functional connectivity (rs-FC), assessed by functional connectivity magnetic resonance imaging (fcMRI), in the amygdala-PFC circuit. In females only, greater ELS predicted increased childhood cortisol levels, which predicted decreased amygdala-vmPFC rs-FC 14 years later. For females, adolescent amygdala-vmPFC functional connectivity was inversely correlated with concurrent anxiety symptoms but positively associated with depressive symptoms, suggesting differing pathways from childhood cortisol levels function through adolescent amygdala-vmPFC functional connectivity to anxiety and depression. These data highlight that, for females, the effects of ELS and early HPA-axis function may be detected much later in the intrinsic processing of emotion-related brain circuits.

What types of stress can cause pregnancy problems?

Stress is not all bad. When you handle it right, a little stress can help you take on new challenges. Regular stress during pregnancy, such as work deadlines and sitting in traffic, probably don’t add to pregnancy problems.

However, serious types of stress during pregnancy may increase your chances of certain problems, like premature birth. Most women who have serious stress during pregnancy can have healthy babies. But be careful if you experience serious kinds of stress, like:

  • Negative life events. These are things like divorce, serious illness or death in the family, or losing a job or home. 
  • Catastrophic events. These are things like earthquakes, hurricanes or terrorist attacks. 
  • Long-lasting stress. This type of stress can be caused by having financial problems, being abused, having serious health problems or being depressed. Depression is medical condition where strong feelings of sadness last for long periods of time and prevent a person from leading a normal life. 
  • Racism. Some women may face stress from racism during their lives. This may help explain why African-American women in the United States are more likely to have premature and low-birthweight babies than women from other racial or ethnic groups. 
  • Pregnancy-related stress. Some women may feel serious stress about pregnancy. They may be worried about miscarriage, the health of their baby or about how they’ll cope with labor and birth or becoming a parent. If you feel this way, talk to your health care provider.

Does post-traumatic stress disorder affect pregnancy?
Post-traumatic stress disorder (PTSD) is when you have problems after seeing or experiencing a terrible event, such as rape, abuse, a natural disaster, a terrorist attack or the death of a loved one. People with PTSD may have:

  • Serious anxiety 
  • Flashbacks of the event 
  • Nightmares 
  • Physical responses (like a racing heartbeat or sweating) when reminded of the event

As many as 8 in 100 women (8 percent) may have PTSD during pregnancy. Women who have PTSD may be more likely than women without it to have a premature or low-birthweight baby. They also are more likely than other women to have risky health behaviors, such as smoking cigarettes, drinking alcohol or taking street drugs. Doing these things can increase the chances of having pregnancy problems. If you think you may have PTSD, talk to your provider or a mental health professional.

How does stress cause pregnancy problems?
We don’t completely understand the effects of stress on pregnancy. But certain stress-related hormones may play a role in causing certain pregnancy complications. Serious or long-lasting stress may affect your immune system, which protects you from infection. This can increase the chances of getting an infection of the uterus. This type of infection can cause premature birth.

Stress also may affect how you respond to certain situations. Some women deal with stress by smoking cigarettes, drinking alcohol or taking street drugs, which can lead to pregnancy problems.

Can high levels of stress in pregnancy hurt your baby later in life?
Some studies show that high levels of stress in pregnancy may cause certain problems during childhood, like having trouble paying attention or being afraid. It’s possible that stress may also affect your baby’s brain development or immune system.

How can you reduce stress during pregnancy?
Here are some ways to reduce stress:

  • Figure out what’s making you stressed and talk to your partner, a friend or your health care provider about it. 
  • Know that the discomforts of pregnancy are only temporary. Ask your provider how to handle these discomforts. 
  • Stay healthy and fit. Eat healthy foods, get plenty of sleep and exercise (with your provider’s OK).
  • Exercise can help reduce stress and also helps prevent common pregnancy discomforts. 
  • Cut back on activities you don’t need to do. 
  • Have a good support network, including your partner, family and friends. Ask your provider about resources in the community that may be able to help. 
  • Ask for help from people you trust. Accept help when they offer. For example, you may need help cleaning the house, or you may want someone to go with you to your prenatal visits. 
  • Try relaxation activities, like prenatal yoga or meditation. 
  • Take a childbirth education class so you know what to expect during pregnancy and when your baby arrives. Practice the breathing and relaxation techniques you learn in your class. 
  • If you’re working, plan ahead to help you and your employer get ready for your time away from work. 
  • If you think you may be depressed, talk to your provider right away. There are many ways to deal with depression. Getting treatment and counseling early may help.

Both the culture and the economy are experiencing turmoil. For some communities, the unsettled environment is a new phenomenon, for other communities, children have been stressed for generations. According to the article, Understanding Depression which was posted at the Kids Health site:

Depression is the most common mental health problem in the United States. Each year it affects 17 million people of all age groups, races, and economic backgrounds.

As many as 1 in every 33 children may have depression; in teens, that number may be as high as 1 in 8.

Schools are developing strategies to deal with troubled kids.

Anna M. Phillips has written the New York Times article, Calming Schools by Focusing on Well-Being of Troubled Students which describes how one New York school is dealing with its troubled children.

Mark Ossenheimer, principal of the Urban Assembly School for Wildlife Conservation in the Bronx, threw out a name to add to the list of teenagers in trouble.

Several teachers and a social worker seated around a table in the school’s cramped administrative offices nodded in agreement. They had watched the student, who had a housebound parent who was seriously ill, sink into heavy depression. Another child seemed to be moving from apartment to apartment, showing up at school only sporadically. And then there was the one grappling with gender-identity issues. Soon the list had a dozen names of students who could shatter a classroom’s composure or a school windowpane in a second.

Convening the meeting was Turnaround for Children, a nonprofit organization that the young-but-faltering school in an impoverished neighborhood near the Bronx Zoo had brought in this year to try to change things.

This is the condition our organization was created to solve,” said Dr. Pamela Cantor, Turnaround’s founder and president. “A teacher who works in a community like this and thinks that these children can leave their issues at the door and come in and perform is dreaming.”

In focusing on students’ psychological and emotional well-being, in addition to academics, Turnaround occupies a middle ground between the educators and politicians who believe schools should be more like community centers, and the education-reform movement, with its no-excuses mantra. Over the past decade, the movement has argued that schools should concentrate on what high-quality, well-trained teachers can achieve in classrooms, rather than on the sociological challenges beyond their doors.

The stress of a spelling bee or a challenging science project can enhance a student’s focus and promote learning. But the stress of a dysfunctional or unstable home life can poison a child’s cognitive ability for a lifetime, according to new research.

While educators and psychologists have said for decades that the effects of poverty interfere with students’ academic achievement, new evidence from cognitive and neuroscience is showing exactly how adversity in childhood damages students’ long-term learning and health.

Those studies show that stress forms the link between childhood adversity and poor academic achievement, but that not all adversity—or all stress—is bad for students….

Compounding Risks

As part of the Adverse Childhood Experiences Study, more than 17,400 adults in San Diego County were assigned scores based on the number of risk factors each experienced as a child, including abuse or neglect, or growing up in homes with domestic violence, drug abuse or mental illness, or absent parents. Researchers found that people with higher ACE scores were more likely to experience underage sex or pregnancy, among other health risks.

Research from Dr. Shonkoff’s center and from other experts finds that positive stress—the kind that comes from telling a toddler he can’t have a cookie or a teenager that she’s about to take a pop quiz—causes a brief rise in heart rate and stress hormones. A jolt can focus a student’s attention and is generally considered healthy.

Similarly, a child can tolerate stress that is severe but may be relatively short-term—from the death of a loved one, for example—as long as he or she has support….

‘Toxic’ Recipe

By contrast, so-called “toxic stress” is severe, sustained, and not buffered by supportive relationships.

The same brain flexibility, called plasticity, that makes children open to learning in their early years also makes them particularly vulnerable to damage from the toxic stressors that often accompany poverty: high mobility and homelessness; hunger and food instability; parents who are in jail or absent; domestic violence; drug abuse; and other problems, according to Pat Levitt, a developmental neuroscientist at the University of Southern California and the director of the Keck School of Medicine Center on the Developing Child in Los Angeles.

The exponential brain growth of infancy and early childhood also makes children more vulnerable to chronic stress during those years than at other developmental periods, according to the National Scientific Council on the Developing Child, an interdisciplinary group of neuroscientists, psychologists, economists, and education researchers. In a series of easy-to-understand, peer-reviewed videos, the group explains how early cognitive connections form—and break down.

Good experiences, like nurturing parents and rich early-child-care environments, help build and reinforce neural connections in areas such as language development and self-control, while adversity weakens those connections.

Over time, the connections, good or bad, stabilize, “and you can’t go back and rewire; you have to adapt,” Dr. Shonkoff said. “If you’ve built on strong foundations, that’s good, and if you have weak foundations, the brain has to work harder, and it costs more to the brain and society.”
