T cell: A type of white blood cell that is of key importance to the immune system and is at the core of adaptive immunity, the system that tailors the body’s immune response to specific pathogens. The T cells are like soldiers who search out and destroy the targeted invaders.

Immature T cells (termed T-stem cells) migrate to the thymus gland in the neck, where they mature and differentiate into various types of mature T cells and become active in the immune system in response to a hormone called thymosin and other factors. T-cells that are potentially activated against the body’s own tissues are normally killed or changed (“down-regulated”) during this maturational process.

There are several different types of mature T cells. Not all of their functions are known. T cells can produce substances called cytokines such as the interleukins which further stimulate the immune response. T-cell activation is measured as a way to assess the health of patients with HIV/AIDS and less frequently in other disorders.

T cell are also known as T lymphocytes. The “T” stands for “thymus” — the organ in which these cells mature. As opposed to B cells which mature in the bone marrow.

T Cell Types

T cells are one of three main types of lymphocytes. The other types include B cells and natural killer cells. T cell lymphocytes are different from B cells and natural killer cells in that they have a protein called a T-cell receptor that populates their cell membrane. T-cell receptors are capable of recognizing various types of specific antigens (substances that provoke an immune response). Unlike B cells, T cells do not utilize antibodies to fight germs.

There are several types of T cell lymphocytes, each with specific functions in the immune system. Common T cell types include:

Although this treatment can effectively eliminate tumors, the ‘training’ of the T cells is complex and expensive. “Sometimes, the trained T cells are not highly potent because the patient already received a number of treatments that weakened the immune cells we work with,” Mamonkin said.

In addition, the process to manufacture the therapeutic T cells is time consuming. “Sometimes it takes weeks to get the T cells ready, and in this time the patient may take a turn for the worse,” Mamonkin said.

The next step: off-the-shelf therapies

“Now that we know that this type of cell immunotherapy has a lot of promise, the next step is to streamline it, make it more accessible and make sure that the resulting T cells have the highest potency,” said Mamonkin, who also is a member of the Dan L Duncan Comprehensive Cancer Center.

“This approach solves two limitations of the original approach: it avoids the time-consuming, elaborate steps of training and expanding the patient’s cells and results in therapeutic T cells of higher potency,” Mamonkin said. “However, the novel approach presents a new set of limitations.”

Dealing with rejection

One of the limitations of the off-the-shelf approach emerges when the therapeutic T cells enter the patient’s body. The patient’s own immune system recognizes the cells as foreign, such as it happens with organ transplants, and may reject the therapeutic cells.

“This is a major problem because rejection not only would reduce the duration of the T cells activity against the tumor, but also would preclude giving subsequent doses of cells. The immune system would reject subsequent doses of the cells right way,” said first author, Feiyan Mo, graduate student in Mamonkin’s lab. “To solve this problem we thought that the best defense was a good offense.” The researchers gave the therapeutic T cells a tool that would enable them to fight back the attack of the patient’s immune cells against them. They genetically engineered the therapeutic T cells to express a receptor called alloimmune defense receptor, or ADR. ADR recognizes a specific molecule, called 4-1BB, that is only expressed on the patient’s activated T cells and natural killer (NK) cells that would attack them. 4-1BB is not expressed on resting T and NK cells that do not turn against the therapeutic T cells.

“Both experiments in the lab and animal models with blood cancers or solid tumors showed that ADR protected off-the-shelf therapeutic T cells from being rejected,” Mo said. “Not only did they resist rejection, but they also expanded more and persisted longer than therapeutic T cells without ADR.” The researchers are optimistic that this approach may also work in patients. They plan to conduct clinical trials on 2021.

Beyond cancer applications

“If successful, this approach can be extended to targeting other disease-causing T-cells, such as those rejecting transplanted organs, mediating graft-versus-host disease or perpetuating autoimmunity,” said Mamonkin. “We are very excited to develop this concept for several applications beyond cancer therapy.” This technology has been licensed to Fate Therapeutics, a clinical-stage biopharmaceutical company that plans on integrating ADR into their clinical products.

“The BCM Ventures team is very pleased to partner with Fate Therapeutics in a licensing relationship to support their implementation of the ADR technology developed in the Mamonkin laboratory here at BCM. This approach promises to enhance the effectiveness of off-the-shelf cell therapies, and it will now be used more extensively in the clinical setting which stands to benefit patients,” said Michael Dilling, director of Baylor Licensing Group. “BCM has been an innovator in the development of cell therapies and the commercial sector increasingly looks to BCM as a source for new innovations.”

Feiyan Mo, who took the lead on this work, has received an NIH NCI F99/F00 Predoctoral-to-postdoctoral Fellowship Award to help facilitate the translation of ADR to the clinic and continue postdoctoral studies in cancer biology. T-cell transfer therapy is a type of immunotherapy that makes your own immune cells better able to attack cancer. There are two main types of T-cell transfer therapy: tumor-infiltrating lymphocytes (or TIL) therapy and CAR T-cell therapy. Both involve collecting your own immune cells, growing large numbers of these cells in the lab, and then giving the cells back to you through a needle in your vein. T-cell transfer therapy is also called adoptive cell therapy, adoptive immunotherapy, and immune cell therapy.

The process of growing your T cells in the lab can take 2 to 8 weeks. During this time, you may have treatment with chemotherapy and, maybe, radiation therapy to get rid of other immune cells. Reducing your immune cells helps the transferred T cells to be more effective. After these treatments, the T cells that were grown in the lab will be given back to you via a needle in your vein.

  • TIL therapy uses T cells called tumor-infiltrating lymphocytes that are found in your tumor. Doctors test these lymphocytes in the lab to find out which ones best recognize your tumor cells. Then, these selected lymphocytes are treated with substances that make them grow to large numbers quickly.

The idea behind this approach is that the lymphocytes that are in or near the tumor have already shown the ability to recognize your tumor cells. But there may not be enough of them to kill the tumor or to overcome the signals that the tumor is releasing to suppress the immune system. Giving you large numbers of the lymphocytes that react best with the tumor can help to overcome these barriers.

  • CAR T-cell therapy is similar to TIL therapy, but your T cells are changed in the lab so that they make a type of protein known as CAR before they are grown and given back to you. CAR stands for chimeric antigen receptor. CARs are designed to allow the T cells to attach to specific proteins on the surface of the cancer cells, improving their ability to attack the cancer cells.

What cancers are treated with T-cell transfer therapy?

T-cell transfer therapy was first studied for the treatment of metastatic melanoma because melanomas often cause a strong immune response and often have many TILs. The use of TIL therapy has been effective for some people with melanoma and has produced promising findings in other cancers, such as cervical squamous cell carcinoma and cholangiocarcinoma. However, this treatment is still experimental.

Two CAR T-cell therapies have been approved by the Food and Drug Administration, both for blood cancers:

CAR T-cell therapy has also been studied for the treatment of solid tumors, including breast and brain cancers, but use in such cancers is still experimental.

What are the side effects of T-cell transfer therapy?

T-cell transfer therapy can cause side effects, which people experience in different ways. The side effects you may have and how serious they are will depend on how healthy you are before treatment, your type of cancer, how advanced it is, the type of T-cell transfer therapy you are receiving, and the dose.

Doctors and nurses cannot know for sure when or if side effects will occur or how they will affect you. So, it is important to know which signs to look for and what to do if you start to have problems.

CAR T-cell therapy can cause a serious side effect known as cytokine release syndrome. This syndrome is caused when the transferred T cells, or other immune cells responding to the new T cells, release a large amount of cytokines into the blood.

Cytokines are immune substances that have many different functions in the body. A sudden increase in their levels can cause:

  • Fever
  • Nausea
  • Headache
  • Rash
  • Rapid heartbeat
  • Low blood pressure
  • Trouble breathing

Most patients have a mild form of cytokine release syndrome, but in some people it may be severe or life threatening.

Also, although CAR T cells are designed to recognize proteins that are found only on cancer cells, they can also sometimes recognize normal cells. Depending on which normal cells are recognized, this can cause a range of side effects, including organ damage.

TIL therapy can cause capillary leak syndrome. This syndrome causes fluid and proteins to leak out of tiny blood vessels and flow into surrounding tissues, resulting in dangerously low blood pressure. Capillary leak syndrome may lead to multiple organ failure and shock.

Baylor University study: ‘Violence Free Zone’ program can be effective

The Centers for Disease Control (CDC) collects statistics about school violence. According to School Violence: Data & Statistics, the CDC reports:

The first step in preventing school violence is to understand the extent and nature of the problem. The Centers for Disease Control and Prevention (CDC), the U.S. Department of Education, and the U.S. Department of Justice gather and analyze data from a variety of sources to gain a more complete understanding of school violence.
According to the CDC’s School Associated Violent Death Study, between 1% and 2% of all homicides among school-age children happen on school grounds or on the way to and from school or during a school sponsored event. So the vast majority of students will never experience lethal violence at school.1

A Baylor University study examined an intervention strategy which might be effective in reducing school violence.

Science Daily reports in ‘Violence-free’ zones improve behavior, performance in middle, high school students:

A youth violence-reduction mentoring program for trouble-plagued schools in urban centers has contributed to improved student behavior and performance at high-risk middle and high schools in Wisconsin and Virginia, according to a new Baylor University case study.

The “Violence-Free Zone” is the national model of mentoring students in areas with high levels of crime and violence. The mentoring program is designed to address behaviors that result in truancies, suspensions, violent incidents, involvement in drugs and gangs and poor academic performance in public middle and high schools.

Four evaluations of VFZ programs conducted between 2007 and 2013 show positive impact, including a unique return-on-investment (ROI) analysis of a VFZ high school in Milwaukee, according to study leaders Byron Johnson, Ph.D., director of the Program on Prosocial Behavior in Baylor’s Institute for Studies of Religion, and William Wubbenhorst, non-resident fellow at Baylor and scholar in faith-based and community initiatives.

The case study evaluates improvements at two VFZ high schools in Richmond, as well as the impact of the Milwaukee VFZ program on youths mentored by adults who work full time in the schools as hall and cafeteria monitors and role models. They work closely with safety officers, teachers and counselors.
Among the key findings:

1. A four-year study (academic years 2007 to 2010) of the VFZ Program in Milwaukee’s School for Career and Technical Education showed a: • 44 percent reduction in the average number of behavioral incidents per VFZ student per month • 79 percent reduction in average number of suspension days per VFZ student per month • 23 percent reduction in truancy incidents per VFZ student per month • 9.3 percent increase in GPA per VFZ student • 24 percent higher rate of graduation from high school than non-VFZ students • 8 percent higher college enrollment rate (as compared to the Wisconsin state level) • 64 percent increase in the number of students reporting a more positive school climate (as compared to the year prior to the VFZ program start)

2. A Return-On Investment Analysis of the Milwaukee school’s program showed an estimated lifetime savings of $8.32 for every $1 invested in the VFZ program, based on reduced administrative costs from fewer suspensions; reduced police costs from service calls; reduced juvenile detention costs; lower truancy rates; savings from reduced number of auto thefts within 1,000 feet of the school; savings from reductions of such high-risk behaviors as drinking, violence against intimate partners or violence against oneself; and projected increases in lifetime earning associated with higher high school graduation and college enrollment rates.

3. A four-year study (from academic years 2009-2012) of overall school-level trends of the VFZ program in Richmond showed a: • 44 percent reduction in the average number of suspensions per student • 27 percent reduction in the average number of suspension days per student • 18 percent increase in the average grade point average

4. A one-year study (academic year 2013-14) of VFZ students in three middle schools and eight high schools in Milwaukee showed a: • 7 percent decrease in the average number of non-violent incidents per VFZ student per month • 31 percent decrease in the average number of violent incidents per VFZ student per month.
The Milwaukee Violence-Free Zone program was created and is directed by the Washington, D.C.-based Center for Neighborhood Enterprise and implemented in Milwaukee schools by CNE’s community partners, Running Rebels Community Organization and the Milwaukee Christian Center. The Richmond program was operated in partnership with the Richmond Outreach Church.

A Proven Model for Stopping Violence in the Schools and Creating Peace in the Community
The Violence-Free Zone is the national model of a youth violence reduction and high-risk- student mentoring program created by the Center for Neighborhood Enterprise. Designed to operate in the most trouble-plagued schools in urban centers with high levels of crime and violence, the VFZ has produced measurable decreases in violent and non-violent incidents and suspensions in more than 30 schools across the country. The principles developed in the Violence-Free Zone model have also proved applicable to suburban and rural communities.
Three studies by evaluators from Baylor University reported that the VFZ had measurable impact in improved safety, reduction in suspensions and truancies, and increased academic performance. Educators and law enforcement officers from sites around the country have praised the VFZ for changing the culture of previously violent schools and reducing crimes in surrounding neighborhoods.
How It Works
The goal of the Violence-Free Zone initiative is to reduce violence and disruptions in the schools and prepare students for learning. The Center provides overall management and direction to the Violence-Free Zone initiative sites, and selects established youth-serving organizations to be CNE’s community partners and implement the VFZ program in the schools. These organizations have the goal of stopping violence in their neighborhoods and have demonstrated that they have the trust and confidence of young people. The Center provides training in the Violence-Free Zone national model as well as technical assistance, administrative and financial oversight, and linkages to sources of support.
Central to the program are the Youth Advisors, mature young adults who are from the same neighborhoods as the students in the schools they serve. The Youth Advisors command respect because they have faced and overcome the same challenges as the students. WACO, Texas (March 23, 2015) — A youth violence-reduction mentoring program for trouble-plagued schools in urban centers has contributed to improved student behavior and performance at high-risk middle and high schools in Milwaukee, Wisconsin, and Richmond, Virginia, according to findings of a new Baylor University case study.
The “Violence-Free Zone” (VFZ) is the national model of mentoring students in areas with high levels of crime and violence. The VFZ mentoring program is designed to address behaviors that result in truancies, suspensions, violent incidents, involvement in drugs and gangs and poor academic performance in public middle and high schools.
Four evaluations of VFZ programs conducted between 2007 and 2013 show positive impact, including a unique return-on-investment (ROI) analysis of a VFZ high school in Milwaukee, according to study leaders Byron Johnson, Ph.D., director of the Program on Prosocial Behavior in Baylor’s Institute for Studies of Religion, and William Wubbenhorst, non-resident fellow at Baylor, scholar in faith-based and community initiatives and co-president of Social Capital Valuations, LLC.
The case study also includes an evaluation of school-level improvements at two VFZ high schools in Richmond, as well as the impact of the Milwaukee VFZ program specifically on youths directly receiving mentoring services from the VFZ “Youth Advisers” — adults who work full time in the schools as hall and cafeteria monitors, role models and mentors. They work closely with school safety officers, teachers and counselors to provide a support system for students.
Among the key findings:
1. A four-year study (academic years 2007 to 2010) of the VFZ Program in Milwaukee’s School for Career and Technical Education showed a:
44 percent reduction in the average number of behavioral incidents per VFZ student per month
79 percent reduction in average number of suspension days per VFZ student per month
23 percent reduction in truancy incidents per VFZ student per month
9.3 percent increase in GPA per VFZ student
24 percent higher rate of graduation from high school than non-VFZ students
8 percent higher college enrollment rate (as compared to the Wisconsin state level)
64 percent increase in the number of students reporting a more positive school climate (as compared to the year prior to the VFZ program start)
2. A Return-On Investment Analysis of the Milwaukee school’s program showed an estimated lifetime savings of $8.32 for every $1 invested in the VFZ program, based on reduced administrative costs from fewer suspensions; reduced police costs from service calls; reduced juvenile detention costs; lower truancy rates; savings from reduced number of auto thefts within 1,000 feet of the school; savings from reductions of such high-risk behaviors as drinking, violence against intimate partners or violence against oneself; and projected increases in lifetime earning associated with higher high school graduation and college enrollment rates.
3. A four-year study (from academic years 2009-2012) of overall school-level trends of the VFZ program in Richmond showed a:
44 percent reduction in the average number of suspensions per student
27 percent reduction in the average number of suspension days per student
18 percent increase in the average grade point average
4. A one-year study (academic year 2013-14) of VFZ students in three middle schools and eight high schools in Milwaukee showed a:
7 percent decrease in the average number of non-violent incidents per VFZ student per month
31 percent decrease in the average number of violent incidents per VFZ student per month
The Milwaukee Violence-Free Zone program was created and is directed by the Washington, D.C.-based Center for Neighborhood Enterprise (CNE) and implemented in Milwaukee schools by CNE’s community partners, Running Rebels Community Organization and the Milwaukee Christian Center. The Richmond program was operated in partnership with the Richmond Outreach Church.
